The Accu-Chek Performa test strip is intended to be used with the Accu-Chek Performa blood glucose meters to quantitatively measure glucose in fresh venous, arterial, neonatal, and capillary whole blood from the finger as an aid in monitoring the effectiveness of glucose control. The Accu-Chek Performa test strips, used with Accu-Chek meter provide complete test systems that are meant for in vitro diagnostic use by healthcare professionals in clinical settings and by people with diabetes at home. The systems are not for use in diagnosis or screening of diabetes mellitus, nor for testing neonate cord blood samples. Before a blood glucose test is performed, the meter need to be coded first. Coding Your Meter 1. Make sure the meter is off and turn the meter over. 2. Remove the white code chip (if there is a white code chip in the meter) and discard it. 3. Position the new code chip so the code number faces away from you. Push the code chip into the code chip slot until it snaps into place. 4. Leave the code chip in the meter until you open a new box of test strips. Note: The code chips that come with Accu-Chek Performa test strips are changing from white to black. Once you receive a black code chip, inserts it into the meter. You do not need to change the black code chip again even if new test strip boxes contain different colored code chips or different code numbers. Testing Your Blood Glucose 1. Wash and dry your hands. 2. Prepare the lancing device. 3. Check the use by date on the test strip container. Do not use test strips past the use by date. 4. Inserts a test strip into the meter in the direction of the arrows. 5. Perform a fingerstick with the lancing device. 6. Gently squeeze your finger to assist the flow of blood. 7. Touch the blood drop to the front edge of the yellow window of the test strip. Do not put blood on top of the test strip. 8. The test result appears on the display. 9. Remove and discard the used test strip.
